46yo male presents with chief complaint of left wrist pain. Patient slipped on side walk on sheet of ice and fell forward, landing on outstretched hand.
Scapholunate Dissociation Patients may complain of a tear or a “pop.” Long-standing cases may present with deformity and/or degenerative arthritis due to carpal instability. Clicking may be present with recurrent subluxation during motion.
Engelska. Scapholunate Xray indikerar scapholunat ligamentinstabilitet när scapholunate avståndet är mer än 3 mm, vilket kallas scapholunate dissociation .

Known as Terry-Thomas sign (or David Letterman sign) Rotary subluxation of the scaphoid. Lateral view: Scapholunate angle >60 degrees. The Terry Thomas (or Madonna) sign is given to the increase in the scapholunate space identified on an AP radiograph. The increased distance indicates scapholunate dissociation. 3 article feature images from this case 16 public playlist includes this case Scapholunate advanced collapse (SLAC) Scapholunate Dissociation. There is increased distance of greater than 5 mm (black arrow) between the scaphoid (S) and the lunate (L) producing the so-called Terry Thomas or David Letterman sign.
SLIL may either be injured alone or in combination with other ligaments such as radioscaphocapitate ligament or dorsal intercarpal ligament.

Lavernia CJ, Cohen MS, Taleisnik J: Treatment of scapholunate dissociation by ligamentous repair and capsulodesis.
Scapholunate dissociation rapidly is becoming the most common form of carpal instability. Despite the literature supporting the importance of early diagnosis and treatment, this problem is not often recognized on initial presentation. Combined fracture of the scaphoid and capitate bones with concurrent scapholunate dissociation, but without severe dislocation, is a rare lesion which results in significant carpal instability and requires operative treatment. We report a case of this unusual injury and its functional result after 20 months.

We have reviewed prospectively 44 cases of chronic scapholunate dissociation treated by Blatt’s dorsal capsulodesis. The diagnosis was based on clinical and arthroscopic criteria. The minimum follow-up was 2 years. The results were analysed clinically and radiologically.
Carpal dissociation implies carpal instability, which has important clinical implications; thus it is essential to identify this finding on imaging. Scapholunate dissociation is the most common and most significant ligamentous injury of the wrist. [1] [2] Scapholunate instability is the most frequent pattern of carpal instability occurring separately and as part of other wrist disorders.
Jun 3, 2014 Scapholunate dissociation is the loss of synchronous motion or normal alignment between the scaphoid and lunate bones usually from
1 The mechanism of injury is typically a fall with the wrist extended and in ulnar deviation. Rupture of the scapholunate ligament is the first event in any sequence of perilunar ligament ruptures. It is the most common pattern of wrist instability. Its characteristics are the dissociation of the scaphoid and the lunate, with flexion of the scaphoid, and extension of the lunate. Scapholunate dissociation is one of the most common forms of carpal instability, which is often overlooked and leads to associated morbidity.
Familiarity with the SL ligament complex is required to understand the various features of SL instability. Scapholunate dissociation is the most common form of wrist instability, defined as “the loss of synchronous motion or normal alignment between the scaphoid and the lunate”. 1 The mechanism of injury is typically a fall with the wrist extended and in ulnar deviation. Scapholunate Ligament Tear Signs, Symptoms and Treatment What is Scapho-lunate ligament injury? The wrist is complex joint consisting of 8 small bones, connected to each other with many strong ligaments. as scapholunate (SL) dissociation in a cadaveric, biomechanical study. Methods.
